1. Proper Identification :
A non-expired identification card or driver’s license issued by a United States Department of Motor Vehicles
A non-expired United States passport
*** Please note, the following may ONLY be used if it includes ALL 4 items : photograph, physical description of the person, signature, and an identifying number.
A valid passport from the applicant’s country of citizenship
A driver’s license issued by another state or by a Canadian or Mexican public agency authorized to issue driver’s licenses
An identification card issued by another state
Military ID
2. You have the documents that you need notarized. Notaries are not authorized to provide legal advice and cannot tell you what form you need notarized.
3. Please fill out your documents beforehand. Except for the signature line, you will sign in front of the notary.
